Pilots have always bitterly resented the label which often masks the true cause of an aircraft accident. The first edition of this book therefore posed a challenge to this concept and aimed to bring about a wider recognition of the diverse factors embraced by the term Pilot Error.
The second edition of this book retains very little of the previous material. This study has evolved into a vehicle for those actively engaged in the pursuit of greater aviation safety and, in particular, for those doing so through the Human Factors discipline which is involved at the very centre of the debate.
Amongst the subjects dealt with in the new chapters are the implications of automation on the flight deck, the syndrome of controlled flight into terrain, how cockpit design deficiencies can contribute to visual illusions and how systems deficiencies can bring about air traffic controller error.
All the contributors are acknowledged experts in their respective fields and the ideas which they set forth in these pages warrant serious consideration by all involved with, and concerned for, the safety of aviation. This means not only the professionals but also those members of the public who entrust themselves to the air transport System.
The Editors:
Ronald Hurst after wartime flying Service with the Royal Air Force entered the British aircraft industry and worked on a number of military and civil aircraft design projects. Since 1975 he has served with the Secretariat of the Commonwealth Air Transport Council where his work is concerned with the development of civil aviation in the member countries. He is also the co-author, of 'Zagreb One Four-Cleared to Co/lide?'. He is an Associate Member of the Royal Aeronautical Society.
Leslie R. Hurst who is Ronald Hurst's son, is an English Solicitor involved in aviation.
